Since EA Sports obtained an exclusive contract with the NFL, Midway was forced to design a football game without a license. Blitz: The League is the first football game in many years to be released without an NFL license. There are 17 fantasy teams in the game, each of which has its unique style and feel, from their personalities to their logos.

The game further differentiates itself from official games by providing the players a presence off the field, frequently involving sex, performance-enhancing substances, and violence. The author of Playmakers, the football-themed soap opera that ESPN was forced to stop due to pressure from the NFL, has contributed to the game.

The fact that each team in Blitz has eight players makes it far simpler to pull off stunning, action-packed movements than in real life. When you pull off impressive actions, your Clash meter fills up, which enables you to perform impossible moves. For example, you can go faster thanks to Bullet Time and make some brutally strong defense tackles. If these tackles occur frequently enough, the players will begin to battle hockey-style, lunging into dangerous tackles. The group that completes the most kits in this mode earns access to the Unleashed game mode, which has even more horrible actions and is modeled after a play style that the NFL would never allow to be linked with one of its products.

Additional features include multiplayer action with head-to-head online play and the capabilities for ultimately customizing players, teams, stadiums, and other aspects of the game. Lawrence Taylor is being held up as an example of this.
