The Oneechanbara 2 is a full sequel to the first Oneechanbara game and is the third one in the series.

After having finally defeated her younger sister Saki but refraining from mortally wounding her, Aya takes her to the hospital only to discover her missing after Saki had suffered a strange “fever”. In trying to find her, Aya encounters the mysterious motorcycle helmet-wearing Reiko trying to murder Saki; after stopping her, Aya uncovers a sinister plot.
