MX vs. ATV Alive is the fourth game in the MX vs. ATV series and the follow-up to MX vs. ATV Reflex. The game, like its predecessors, includes a variety of racing modes. For racing, players can use dirt motorcycles and ATVs. Monster trucks, buggies, and airplanes are not permitted in MX vs. ATV Unleashed.

There are both multiplayer and single-player modes in the game. Multiplayer may be played both offline with two individuals split-screen and online with many opponents through the internet. Multiplayer races are divided into three categories: player skill (rookie, amateur, and pro) and MX (dirt bike) or ATV. Amateur races require a player level of 10, while pro races require a group of 25.

Single-player races include a variety of racing tracks, such as riding on snow, sand, or dirt. New courses can be unlocked by progressing through the driving levels, with the highest methods requiring a player level of 25. Before beginning a race, the player can set the opponent’s skill, the number of laps needed, and the vehicle the opponent uses. Accomplishing races, finishing first, making the podium, and accomplishing race goals provide the player with experience, which is utilized to level up. Car experience is gained by using the same car in multiple races to level up that specific vehicle.

Customization is available for all cars and drivers. For example, graphic kits, handlebars, exhausts, tires, wheels, hubs, suspensions, chassis, brakes, and number plates can be added to dirt bikes. Drivers can be personalized by changing their look. Appearance options include three distinct gear sets and the gear of various professional drivers, including Alex Martin, Ashley Fiolek, Chad Wienen, Dakota Tedder, Jeff Alessi, and PJ Larsen.
