The plot and events of Manhunt 2 do not continue or expand upon those of the first game. Instead, the identical game concepts are relocated to a new place, the Dixmor Insane Asylum, where the staff likes to beat up the patients and experiment on them rather than give therapy for their disease. This new setting is where the player will experience the game. One day, a blackout offers the patients the opportunity to rebel and cause mayhem inside the facility. The player can play Dr. Daniel Lamb and Leo Kasper during the game. Although the first few scenes occur within the asylum, the story quickly shifts to other locales.

The stealth-based gameplay is the primary focus of the game’s mechanics; however, later levels transition into more traditional third-person shooter territory as more weapons become accessible. To succeed in gunplay, one must use cover, moves, and sometimes stealth. There is a radar system, different foes have varying levels of awareness, and the easiest way to kill someone is by sneaking up on them from behind. Players on the PlayStation 2 can distract foes by throwing objects at them or making noises through a USB headset to draw them away from their allies. Players can carry out executions in the same three different types as they did in the original game: Hasty (white), Violent (yellow), and Gruesome (red). Other minor adjustments made for the Wii edition include more detailed environments, additional dialogue, new hunters for the Sexual Deviants scene, some modifications to the placement of weapons, a new maintenance hole execution for the Ghosts sequence, and additional animations for Daniel and Leo, particularly during close combat.
